Whatever the details, it's a real tragedy and chilling stuff. We need to wait for whatever full report / court case before confirming anything. However, as others are saying this is why routines and checks are vital on these things, and not left to human chance.

I've found Melon Alleycats really good, but my current faves are Dirtlej Photchromics, which were a half price bargain - and I know they're good, because i don't find myself taking them off, which in turn means I haven't lost them yet (I'm down two pairs of Alleycats)

Even at go ape, it seems to be standard (or was a decade+ back) for one person to get you in the harness and the second person to verify it! At a site in France the heights involved are about 3x that of go ape & they just send you off on your own, instructors aren't even in view all the way ...

I've been out in Spain for the last 5 weeks and there are 1000s of miles of gravel roads and a gravel bike is definitely the tool for the job, I'm probably going to fit some Cane Creek inverts to my Fustle causeway purely because my wrists are knackered after a motorbike accident

There's some chap on YouTube who's just done the WHW on a Brompton* *G-line, but still. And he had some sort of shoulder harness arrangement for the inevitable portage.
